Лечение на пациенти с антикоагулантна терапия в оралната хирургия

Оптималната стратегия при необходимостта от дентално лечение  и извършването на хирургични процедури при нарастващият брой  от пациенти, приемащи дългосрочно антикоагуланти е труден въпрос, тъй като документираният тромботичен риск от спиране на терапията трябва да бъде балансиран с предполагаемия хеморагичен риск от непрекъснато антитромботично лечение. Има доказателства, че временното прекъсване на антитромботичната терапия може да увеличи риска от тромбоемболизъм, причинявайки значителна заболеваемост или дори смъртност за пациентите. Настоящите насоки обикновено предлагат поддържане на антитромботично лечение периоперативно по време на  хирургичната интервенция, тъй като те обикновено се считат за носители на риск от тежки или животозастрашаващи усложнения от кървене, ако не са взети локални хемостатични мерки. Оралната хирургия обаче може да включва спектър от прости интервенции, като например просто зъбно екстрахиране, до по-сложни, като например множество, комплексни или хирургични екстракции или разширени процедури за имплантиране или костно аугментиране, които носят различни рискове от кървене. Следователно, решението за поддържане или прекратяване на антитромботиците преди операция за дентални импланти трябва да бъде внимателно и индивидуално оценено, и да се основава на факторите, свързани с пациента и хирургията. В настоящата лекция ще бъдат обсъдени горепосочените въпроси и ще бъдат представени актуални препоръки за дентално лечение.

Dr Theodoros Lillis, DDS, MSc(med), MSc(dent), PhD, Assistant Professor

Dr. Theodoros Lillis graduated from School of Dentistry of Aristotle University of Thessaloniki (AUTh), Greece in 2007. During 2008 – 2012, he specialized in “Oral Surgery, Implantology and Oral Radiology” in School of Dentistry of AUTh and attended postgraduate program in “Medical Research Methodology” in School of Medicine of AUTh. In 2019 he completed his PhD thesis on the effect of antiplatelet drugs in bone healing and in osseointegration of titanium implants in the Orthopedics Department of School of Medicine of University of Thessaly, Greece. In 2022, he was elected Assistant Professor of Oral Surgery in the School of Dentistry of AUTh. Since 2016, he is Secretary-General of the Hellenic Dental Society of Oral Surgery. His scientific area of interest includes oral surgery, dental implantology, dental anesthesiology and dental management of the medically compromised patient. 

LECTURE TITLE: Management of patients on antithrombotic medication in oral surgery

Summary 

The optimal treatment strategy for dental and oral surgical procedures in the expanding population of patients receiving long-term antithrombotic therapy is a challenging issue, since the documented thrombotic risk of antithrombotic withdrawal needs to be balanced against the putative hemorrhagic risk of uninterrupted antithrombotic treatment. There is evidence that temporal discontinuation of antithrombotic therapy may increase the risk of thrombo-embolism causing significant morbidity or even mortality for patients. Current guidelines generally suggest maintenance of antithrombotic medication perioperatively during oral surgery, since they are generally considered to carry a low risk of severe or life-threatening bleeding complications, if local hemostatic measures have been taken. However, oral surgery may include a spectrum from simple interventions such as simple dental extraction, to more complex ones such as multiple, complex or surgical extractions or extended implant or bone augmentation procedures, which carry different bleeding risks. Thus, the decision to maintain or discontinue antithrombotics prior to dental implant surgery must be carefully and individually assessed, and based on patients’ and surgical factors. In the present lecture the above issues will be discussed and current recommendations for dental management will be presented.
