The All-on-4 Concept: A Predictable and Efficient Solution for Full-Arch Rehabilitation.


The All-on-4 concept, pioneered by Dr. Paulo Malo, is a well-established and innovative treatment for fully edentulous patients, especially those with severe bone resorption. This approach provides a fixed, full-arch prosthesis supported by only four implants per arch, strategically placed to maximize available bone while avoiding complex procedures such as sinus lifts or nerve repositioning.


 


The technique involves placing two anterior implants in an axial position and two posterior implants tilted up to 45 degrees to bypass anatomical limitations, such as the inferior alveolar nerve in the mandible and the maxillary sinus in the upper jaw. This tilted placement increases implant stability, reduces the need for bone grafting, and enables immediate loading in most cases, significantly improving patient experience and treatment efficiency.

          4.       Clinical Outcomes & Success Rates:


          •         Review of clinical studies demonstrating high survival rates and patient satisfaction.


          •         Managing complications and potential risks.


 


This lecture will provide an evidence-based overview of the All-on-4 concept, integrating surgical, prosthetic, and biomechanical perspectives to help clinicians optimize outcomes for fully edentulous.
