Sinus-lifting in cases of inflammatory changes in basal parts of maxillary sinus


The sinus-lifting operation, proposed in 1976 by H. Tatum, became the reliable and predictable procedure of bone augmentation in posterior part of maxilla for dental implant installation. Nevertheless, according to modern literature, intraoperative perforations of the Schneiderian membrane occurs in 10-59.8% of cases. At the same time there are several methods, proposed by different authors, which allow to close these perforations and successfully complete sinus-lifting and sub antral augmentation. We developed our classification of Schneiderian membrane defects and offered intraoperative methods of their closure. Based on the successful application of these methods, we have expanded the possibilities of sinus lifting. In the presence of local chronic inflammation in the form of cysts, polyps or foreign bodies: fragments of the roots of teeth or filling material, we proposed to eliminate them through artificially created perforations of the Schneider membrane. Then the membrane defect is closing and sub antral grafting can be realized. A prerequisite for such manipulations is the patency of the ostiomeatal complex and the absence of fungal lesions of the maxillary sinus.
