DENTISTRY IN PANDEMICS


Syncope in dental practice.


According to the 2018 European Society of Cardiology guidelines, syncope or fainting spells are a transient loss of consciousness due to brain hypoperfusion characterized by rapid development, short duration and spontaneous resolution.
Today, in time of the COVID-19 pandemic, cases of syncopal events are steadily increasing. This is due to both the effect of the virus on the autonomic nervous system and organic damage to cardiomyocytes, which initially leads to edema, and then to myocardial fibrosis, violation of the systolic function of both the left and right ventricles, as well as a violation of the rhythm and conduction of the heart. Special attention should be paid to a group of diseases leading to cardiac arrhythmias, in particular myocarditis, which in its turn occurs in patients after a coronavirus infection.
In dental practice fainting spells are a fairly common problem. Most often, the etiology of these syncopal phenomena is based on vasovagal disorders, particularly, the reason is the fear of dental treatment. However, in terms of the COVID-19 pandemic, a common cause of syncope is rhythm disturbances as a result of cardiac complications after the infection, which requires special control and attention from the dentist to this group of patients.
Every dentist should be able to recognize the syncope and provide first aid, which is of particular importance in the context of a coronavirus pandemic.
When the doctor knows what to do in case of an emergency, he can not only help the patient at the moment, but also determine his further prognosis.
